Right now I've been able to trigger a bug pretty consistently when leaving my burrow by dragging items into containers or things in my inventory. For example, by smelting ingots by combining scrap metal, or bathing myself, I get this when I leave my burrow (lightly edited for privacy since the line of sight is broken):
